How to Approve or Reject a Session Note

Once your staff have completed and signed their session notes, it will be visible in the "Pending Notes" tab.  Any notes unsigned by the note author will reside in the "Unsigned" category.  Notes ready to be reviewed for rejection or approval will be in the "Awaiting Approval" category.



To review the notes, click on "Awaiting Approval" and then click on the note you want to review. 

Read through the note to determine if it meets your organization's standards.  To approve or reject a note, click on "Approve" or "Reject" in the top right-hand corner of the grey bar.


If a note is rejected, the rejection form will populate requesting more information pertaining to why this note is not meeting criteria.  In this example, we rejected the note because the session times did not match the appointment time. We have also included feedback in the text box for the staff. 

 

After you have filled out the rejection form, click "Submit".  Upon submitting the rejection, the note will be unsigned giving its author an opportunity to fix the issues and re-sign.  

Staff members will be alerted to this rejection by seeing an increase in their Pending Notes total.  When they click on the Pending Notes tab, they will see a Revision Requested category.  They will click on this to view any notes that require revision.

Once they have clicked on the note to be revised, they will see the rejection form and the reasons why the note has been rejected. 

The Note Author can then make corrections and resign the note. Once it has been signed again, it will go back to "Awaiting Approval". You can review the session note to ensure the changes you have requested have been made.

 

 

You are now ready to approve the note. To do so, click "Approve"in the top right-hand grey bar. 

A pop-up will appear requesting you to indicate what type of approval you are granting.  If you require a signature to approve the note, the signature field will populate here as well.  Click "Submit".

Once you have clicked "Submit", you will find the approved session note in the"Timelines and Reports" for that learner. Approved notes are indicated by green or gray checkmarks.
