What is Brellium, and how does it work with Motivity?
Brellium is an AI-powered clinical compliance platform that automatically audits clinical documentation. Together with Motivity, the two platforms create a proactive compliance workflow: notes are created and stored in Motivity (your system of record), audited by Brellium against your organization's compliance criteria, and corrected in Motivity before billing risk escalates.
Brellium reviews documentation against payer requirements, compliance standards, documentation quality rules, and billing risk indicators — helping your organization detect issues before claims submission, reduce audit exposure, and replace manual chart review workflows.
What setup is required before the integration works?
Two things need to be configured up front:
Brellium audit configuration. Work with your Brellium deployment strategist to define your organization's audit rules, compliance standards, and payer-specific criteria within the Brellium platform.
Motivity note templates. Ensure your note templates are configured to display all the data you want sent to Brellium. Brellium receives the complete note text — anything included in the template and filled out by the note author will appear in Brellium for review. This includes signatures: a signature line must exist in the template for each signer you want visible in Brellium (see the signature question below).
How does the integration work?
A clinician signs a session note in Motivity Clinical. For All-In-One (AIO) customers, the note must be linked to an appointment to sync. Clinical-only customers do not need an appointment link.
Brellium pulls eligible notes nightly, typically between 1:00–2:00 AM ET, via an automated API sync. Notes are not sent in real time — a note signed today appears in Brellium after the overnight sync.
Brellium audits each note against your configured criteria and displays pass/fail results per criterion via the Brellium portal
Your team is alerted if issues exist, either by email notification from Brellium or directly in the Brellium interface. These settings are customizable to align with your team's preferences. Reach out to your assigned Brellium implementation specialist to configure to your specifications. Alerts may flag things like missing documentation elements, time inconsistencies, compliance gaps, or documentation quality concerns.
Which notes are pulled into Brellium?
A note is eligible only when it is in one of the following statuses:
Awaiting Approval — signed by the primary author (e.g., an RBT) but still pending approval steps such as supervising BCBA sign-off.
Approved / Completed — all approvals and attestations are complete.
Notes that are unsigned or in an active editing state (Pending or Rejected) cannot be pulled. Brellium can see the status of these notes, but not their content, until they are signed again. If a note is deleted, Brellium is notified but never receives the content.
Additionally, for AIO customers, the note must be linked to an appointment.
Note: Which eligible notes Brellium actually audits — and at what point in the note lifecycle — is configurable per organization. Some organizations audit notes as soon as they reach Awaiting Approval; others wait until notes are fully Approved. Adjust this with your Brellium deployment strategist.
Why aren't my notes showing up in Brellium?
Work through this checklist:
Is the note signed? Unsigned notes are never pulled. The note must be in Awaiting Approval or Approved status at the time of the Brellium nightly sync.
Was the note rejected? Rejected notes return to an editing state and their content is not available to Brellium until corrected and re-signed.
(AIO customers) Is the note linked to an appointment? This is the most common cause. Direct care staff may not realize the appointment link is required — if the note isn't linked, it won't sync. Link the note to its appointment and it will be picked up in the next nightly sync.
Has a nightly sync run yet? Notes that reached an eligible status after the last overnight sync won't appear until the next one.
Does your organization filter by status? If your Brellium configuration only audits Approved notes, a note in Awaiting Approval won't appear until it is fully approved.
Was the note already audited? If so, it will not be pulled again after revision. The final approval will be done directly in Motivity.
What should we do when Brellium flags a note?
Corrections happen inside your existing Motivity approval workflow — no separate documentation process is required. Typically:
A BCBA, supervisor, or QA team member reviews the alert from Brellium.
In Motivity Clinical:
If the note is Awaiting Approval, the reviewer rejects the note with the audit findings as the rejection reason—Note: The rejection workflow can be adjusted to include a text box that can have the Brellium findings copy/pasted into. Work with your Motivity CSM and confirm with your Brellium rep to identify the best way to deliver Brellium findings to note authors.
If the note is already Approved, the reviewer removes the Approved status first, then routes it back.
The note author makes the corrections in Motivity Clinical, re-signs, and the note moves back through the normal approval workflow. The note will not be pulled back into Brellium to be re-audited unless specifically configured that way (see below).
One important exception: if the fix requires changing the note template itself (not just the note's content), the correction can't be made by editing the existing note. The author will need to create a new note using the updated template and link it to the original appointment.
Act on alerts quickly. Approved notes are already considered billable, and delayed corrections increase billing compliance risk. Best practice is for QA and clinical leadership to monitor Brellium alerts daily and resolve identified issues before claims submission whenever possible.
Will Brellium re-audit a note after it's corrected?
Not automatically. By default, each note is audited once — the first time Brellium pulls it — and subsequent revisions are not re-pulled.
This has a timing implication worth knowing: if a note is in Awaiting Approval overnight, Brellium audits that version even if a supervisor rejects it the next day. Organizations that rely on a supervisor rejection/QA workflow should consider the timing of their approval process relative to the nightly sync. Re-audit workflows for corrected notes can be configured on a per-customer basis — discuss options with your Brellium representative.
I'm changing my note template. Do I need to do anything?
Yes — tell Brellium before you change your note template in Motivity.
Brellium's audit criteria are configured around the structure of your note template. If you modify your template without notifying Brellium, audit criteria can break or return unexpected results (for example, notes suddenly failing audits that previously passed).
If you've already made template changes without notifying Brellium, don't worry — Brellium can retroactively update their configuration and re-run affected audits. But proactively coordinating template changes with your Brellium contact avoids the disruption entirely.
Troubleshooting tip: If your notes suddenly start flagging oddly in Brellium, ask yourself: Did we recently update our note template? Did we tell Brellium?
Why don't I see a signature in Brellium?
Brellium receives the note content exactly as your template displays it. For a signature to appear in Brellium, the note template must include a signature line for each signer you want captured — including the approver. If your template only includes the author's signature line, only the author's signature will appear. Both typed and drawn signatures are supported, as long as the signature line exists in the template.
This isn't specific to the Brellium integration — it's how Motivity note templates work generally — but it's a common point of confusion when reviewing notes in Brellium.
